I can't believe no one has mentioned George R. R. Martin. He wrote the Fire and Ice saga. The books are less sword-and-magic based, more character, plot, and intrigue based (especially the first one). Don't get me wrong; There is a good bit of fantasy elements, they just serve more as the background. I highly recommend this series, though I must admit I've only finished the second one (they're really long).

Terry Pratchett all the way, followed by George R. R. Martin and J.K. Rowling.

Used to be Paul Stewart a few years ago...don't know if anyone will have heard of him, I think he's mostly in Britain. Now, probably Pratchett. I'd even say myself, but I'm too lazy to write Just forget I said that, okay?

Darren Shan. I know he's horror and fantasy, but I don't care, he rules.
I also love Philip Pullman and Garth Nix.
They are both exceptional writers.
[ BTW, these are my favourite modern fantasy writers. ]

CLAMP, the author group that is probably better known for writing manga fantasy. They started out with simplified seperate, but many stories that people believed were completely unrelated and then they tied them all together in the end. That much work takes many years of their life and they surprised a lot of their devoted readers with more than just great story lines, but with making the majority of them one.
